ABOUT

The HIV CARE Network is a partnership of all people and organizations dedicated to addressing the issues of HIV and AIDS in a region. Networks are comprised of service providers and community members whose goal is to ensure that services are available, accessible and appropriate for HIV+ individuals and families in the region.

Network activities are funded by New York State Department of Health AIDS Institute (NYSDOH AI), and there are 11 Networks around the state.

 

Objectives

The Network carries out this mission through working towards the following objectives:

  • Ensure representation on the Network by entities/individuals who are representative of the full complement of regional stakeholders.
  • Identify populations and subpopulations of individuals and families with HIV disease.
  • Develop and implement strategies to assess service needs of people living with HIV disease and identify barriers to care and gaps in services.
  • Develop and implement mechanisms to identify emerging issues in the region affecting services for people living with HIV disease and develop a community response.
  • Develop a service plan describing populations living with HIV/AIDS in the region.
  • Promote consumer involvement by increasing and retaining the proportion of consumers in the Network.
  • Develop, promote and implement HIV/AIDS educational programs and trainings and/or refer providers and consumers to trainings.
  • Serve as a clearinghouse for updated regional HIV/AIDS information.
  • Provide input to the Ryan White SCSN (State-wide Coordinated Statement of Need) and participate in SCSN activities.
  • Communicate and collaborate with the AIDS Institute on a regular basis and participate in The State-wide AIDS Delivery Consortium (SASDC).

 

Network Structure

The objectives of the Network are accomplished principally through its standing and ad-hoc committees:

Standing Committees:

  1. Executive Committee
  2. Consumer Involvement Committee
  3. Care Coordination Committee
  4. Policy Advocacy and Education Committee

Each Standing Committee is lead by two co chairs who serve for a two year term and who are elected by committee members.

Ad Hoc Committees can be formed as needed.

 

Core Activities

  • Core Activities of the network include:
  • Promotion of a full balance of HIV/AIDS care and services in the Hudson Valley.
  • Identification of populations and subpopulations of individuals and families with HIV disease.
  • Regular assessment of service needs to identify barriers to care and gaps in the service delivery system.
  • Periodic development of a service delivery plan.
  • Promotion of consumer involvement to enable HIV-infected individuals to participate in and inform HIV/AIDS policy and program development for the needs of PLWH/A.
  • Educational and awareness activities for providers and consumers.
  • Participate in the State-wide Coordinated Statement of Need (SCSN).
  • Collaboration with the AIDS Institute.

 

  • Ryan White Title II (now Part B) HIV Care Networks were established in New York State in 1991 to improve the availability and organization of health care and support services for individuals with HIV and their families.
  • In 1996, amendments to the CARE Act eliminated the requirement that states establish a system of consortia.
  • The New York State Department of Health AIDS Institute continues to support the concept and funding of HIV Care Networks throughout the state.
  • AI recognizes the importance of the role of diverse communities in identifying and addressing regional needs and the value of the Networks as a mechanism for community input and a voice for individuals living with HIV/AIDS.
